High Stakes Summary

High Stakes: The Critical Role of Stakeholders in Health Care by David A. Shore (Associate Dean, Associate Dean, Harvard School of Public Health)

The United States health care system is broken. Critics and commentators point to the staggering costs of health care, the shrinking insurance coverage, and the diminishing access that many U.S. citizens have to essential services and treatments. These problems are compounded by a growing realization that the quality of care across the country is uneven at best, with the goal of finding the appropriate kind and level of care proving elusive. High Stakes enters into the health care debates at a critical time, offering an analysis that homes in on factors that account for many of the inefficiencies and shortcomings of our unsystematic system, and putting forth recommendations that are ideologically blind. Using real-world examples to illustrate the fragile state of health care today, Drs. Shore and Kupferberg enlist a powerful analytic frame to bear on these conflicts: stakeholder management. That involves addressing the present system of conflicts, in which key groups in the field pursue their own interests at the risk of the system at large. Insurance providers want to reduce their payments, hospitals want higher reimbursements, patients want access to unlimited services, and no one wants to work together. The fights are constant and financially disastrous, for every dollar earned by one group is one less dollar earned by another. For health care to succeed as an enterprise, key stakeholder groups must come to appreciate their dependence on other groups. More importantly, they must find ways to align their interests and move beyond entrenched behaviors. As the authors point out, all constituency groups must begin by acknowledging that where people stand on an issue depends on where they sit, and that greater stakeholder alignment and engagement are prerequisites for more effective and efficient health care. Drawing equally from both scholarly studies and real world examples, High Stakes offers health care leaders the necessary tools to both map their current stakeholder relationships and fashion concrete steps to produce greater stakeholder engagement, collaboration, and cooperative competition.

High Stakes Reviews

Shore draws on his groundbreaking work building trust within the health care industry to argue convincingly for breaking down barriers to information sharing, adopting open innovation, and for the stakeholders to clearly align themselves in pursuit of improved patient outcomes. High Stakes is an important read for any health care stakeholder with a future. -- Vidar Jorgensen, Chairman, World Health Care Congress I have admired David Shore's work for many years, and he has never disappointed. He has tackled the often underappreciated role of stakeholder alignment and has given us much to think about and much to work with. Must reading for anyone in or aspiring to a leadership role. -- Ed Reilly, President and Chief Executive Officer, American Management Association High Stakes offers the 21st-century approach of how we ought to address challenges in advancing health. The foundation of stakeholder theory should serve everyone in this complex environment with practical evidence and ethically based ideas. As stakeholder engagement is a key ingredient for success, the author's cogent book built upon a quarter century of experience, should benefit all in multiple leadership levels and sectors in the new practice of 'health diplomacy.' Shore and Kupferberg's compelling approach presents opportunities worth adapting and implementing posthaste. -- Scott C. Ratzan, MD, MPA, Vice President, Global Health, Johnson & Johnson; and Editor-in-Chief, Journal of Health Communication: International Perspectives David Shore cogently writes about the importance of 'walking in someone else's shoes' before attempting collaboration or trying to resolve controversial matters. His prescription gives business and political leaders a road map for changing the tone of their dialogue and increasing their effectiveness. -- Karen Ignagni, President and Chief Executive Officer, America's Health Insurance Plans High Stakes is that rare blend of a great mystery novel and a rigorous academic work. The authors take the reader on a creative and exhilarating ride through health care and other worlds, exploring concepts as seemingly diverse as the Gordian knot, the tragedy of the commons the roots of the burning platform and trust. Drawing from highly successful programs such as 'The Forces of Change,' the authors begin to tie together these seemingly diverse threads through compelling case vignettes expressed in an eloquent and elegant way. At the 'mystery's' end, the authors weave the characters, events, and lessons into a compelling denouement. Case closed: We need to try something completely different; focusing on the stakeholders in health care is not only desirable but necessary. -- Gray Ellrodt, MD, Chief Quality Officer and Chair of Medicine, Berkshire Medical Center and Professor of Medicine, University of Massachusetts School of Medicine Shore and Kupferberg offer health care teams a lifeline in this book. Stakeholder theory and understanding the need for partnership are critical elements in the future sustainability of our health care system. This writing gives practical examples that readers can put into motion immediately in their work environments to not only survive, but thrive in the current challenging world of healthcare. -- Trisha Cassidy, Senior Vice President of System Development and Strategy, Loyola University Health System This book is unique in its focus on the analysis of the stakeholders and on how all the healthcare stakeholders' groups compete and are not encouraged to align. This is a very different approach to encouraging new health policies. The book is effective in keeping readers' attention and presenting challenging new thinking. --Doody's
