FRIESEN is Professor of Surgery and the History and Philosophy of Medicine at the University of Kansas. IVAN D.A. JOHNSTON is Professor of Surgery at the University of Newcastle-upon-Tyne. RONALD A. SELLWOOD is Emeritus Professor of Surgery at the University of Manchester.
